Altachart

Terms of Use

Effective October 20, 2026

These Terms of Use ("Terms") apply to your use of the Altachart app and this website (together, "Altachart"). By connecting a health care provider to Altachart or otherwise using it, you agree to these Terms and to our Privacy Policy. If you do not agree, do not use Altachart.

1. What Altachart does

Altachart is a personal health record tool. With your authorization, it downloads a read-only copy of your health records from your providers' patient-access APIs and stores it on your own computer, so you can view, search and organize it. Altachart cannot change your medical record or act in your provider's systems on your behalf.

2. Eligibility and authorization

You must be at least 18 years old. You may connect only records you are legally entitled to access: your own, or those of a person who has authorized you, such as through a patient portal proxy account, guardianship or other legal authority. You are responsible for keeping your patient-portal credentials and the computer where Altachart runs secure.

3. Your providers and their portals

Your health care providers and their technology vendors operate the patient portals and APIs Altachart connects to. They decide what information is available, how long access lasts, and when their systems are available. Their terms continue to apply to your portal account. Altachart is not affiliated with, endorsed by, or sponsored by any health care provider or electronic health record vendor.

4. Not medical advice

Altachart does not provide medical advice, diagnosis or treatment, and it is not a medical device. Records it downloads may be incomplete, out of date, or contain errors made by the source; providers may also withhold some information from patient apps. Always rely on your health care professionals for decisions about your care. If you have a medical emergency, call 911 or your local emergency number.

5. Acceptable use

You agree not to use Altachart to access anyone's records without authorization, to interfere with or overload a provider's systems, to get around any security or access limits, or to break any law.

6. Availability and changes

Altachart is in a limited, invitation-only release. We may change, suspend or stop offering Altachart, in whole or in part, at any time. Records already downloaded to your computer remain yours.

7. Ownership

Your health information belongs to you; these Terms give us no rights in it. The Altachart software, name and website content belong to Altachart. HL7 and FHIR are registered trademarks of Health Level Seven International.

8. Disclaimer of warranties

Altachart is provided "as is" and "as available", without warranties of any kind, express or implied, including warranties of merchantability, fitness for a particular purpose, accuracy and non-infringement. We do not warrant that Altachart will be uninterrupted or error-free, or that any record will be complete or accurate.

9. Limitation of liability

To the fullest extent permitted by law, Altachart will not be liable for any indirect, incidental, special, consequential or punitive damages, or for any loss of data, arising from or related to your use of Altachart. Our total liability for any claim relating to Altachart will not exceed one hundred U.S. dollars (US$100). Some jurisdictions do not allow these limits, so they may not apply to you.

10. Ending your use

You may stop using Altachart at any time by removing it in your providers' patient portals and deleting its files from your computer. We may end your access if you breach these Terms. Sections 4 and 7 through 11 survive.

11. Governing law

These Terms are governed by the laws of the State of Texas, without regard to its conflict-of-laws rules.

12. Changes to these Terms

We may update these Terms. We will post the new version here with a new effective date. If a change is material, we will tell you before it takes effect. Continuing to use Altachart after that means you accept the updated Terms.
