Your health records, on your own computer.
Altachart is a personal health record app. With your permission, it downloads a read-only copy of your medical records from the patient portals of your doctors and hospitals, and keeps it on a computer you control.
Many people see several providers, and each one keeps its own chart in its own portal. Altachart brings those copies together in one place you own, and keeps them up to date. You can then search them, organize them, or share them with a new doctor.
How it works
- You connect a provider. Altachart opens your provider's own patient-portal sign-in page. You sign in there, with your existing portal account. Altachart never sees or stores your portal password.
- You choose what to allow. The portal shows you what Altachart is asking for and lets you pick how long access lasts. You can approve or decline.
- Altachart downloads your records. It uses the provider's standard patient-access API (HL7® FHIR® with SMART on FHIR authorization), the same kind of access federal rules require providers to offer patients.
- It checks for new records. While your access is active, Altachart checks for new or updated records on a schedule and saves only what changed.
What it reads
Altachart requests read-only access to the information your provider makes available to patient apps. That typically includes:
- Visits and encounters, clinical notes, and care plans
- Problems and diagnoses, allergies, and immunizations
- Medications and prescriptions
- Lab results, vital signs, imaging and other reports
- Procedures, care team members, and related provider and location details
- Records your provider received from other organizations, where the provider shares them
Altachart cannot change anything in your medical record, send messages, book appointments, or make payments. Your provider decides which information it shares with patient apps, so a download may not include everything in your full medical record.
Where your data goes
Your records are saved on your computer, not ours. Altachart does not run a cloud service that receives, stores, or analyzes your health information. It does not sell your data, show ads, or share your records with anyone.
The keys that let Altachart keep your access active are stored on your computer, readable only by your user account. Records travel directly from your provider to your computer over encrypted (HTTPS) connections. Details are in our Privacy Policy.
Stopping access
You can stop Altachart's access at any time:
- In your provider's patient portal, open the settings for connected or linked apps and remove Altachart. Altachart can no longer download anything from that provider.
- Access also ends on its own when the period you chose at sign-in runs out.
- To remove records already downloaded, delete them from your computer.
Who can use Altachart
Altachart is in a limited, invitation-only release. It is for adults managing their own records, or for someone a patient has authorized to help manage theirs. Altachart is not a medical device and does not give medical advice. Always talk with your health care provider about your care.
For health care organizations
Altachart is a patient-facing, read-only application. It uses the SMART App Launch standalone flow with PKCE, requests only patient-level read access, and, where the organization supports it, registers a per-patient key so it can renew access without storing a long-lived secret. It does not write data back to the EHR. Questions about the integration: support@altachart.com.
